Be sure to bring a copy of the PARmed-X for Pregnancy form to your visit. This form serves as a medical clearance form and most prenatal fitness programs will require a copy signed by you and your healthcare provider. It considers your past/present health status and activity levels as well as any possible contraindications to exercise.
